The West Orange Health Alliance is releasing a Request for Proposal (RFP) to identify dental care professionals for the provision of dental services to low-income, residents of west Orange County, FL.   

The mission of the West Orange Health Alliance (WOHA) is to promote and protect the health and well-being of those persons residing in the western portion of Orange County, FL, through the provision and accessibility of health and dental care, health education, disease prevention, wellness and other programs to meet the needs of the citizens; with a particular emphasis on the under-served, uninsured, under insured and low-income population.
A one-page Letter of Intent must include the following:
   -Business Name
   -Business Address
   -Address where proposed services will be provided (if different than
       business address)
   -Project Manager/Contact Person name, email address and phone number
   -Proposed Name of Program
   -Provide a brief, one-two paragraph summary of the proposed project and
      services to be provided.  Services can include provision of dental care
      at established dental practices that offer time slots for participants of
      this program; establishing a full or part-time dental practice in West
      Orange County that target low-income, uninsured residents; other creative,
      innovative programs and initiatives are encouraged

Letter of intent must be postmarked by April 15, 2016 and mailed to:

Following a review of all Letters of Intent that were received in a timely manner, selected candidates will be invited to submit a full proposal.  Candidates selected to submit a full proposal will be notified on or before May 1, 2016. 

Full proposals are due May 31, 2016.  Awards (grant cycle) will begin on July 1, 2016 for a period of one year.   


Awards can range from $10,000 to $300,000.  Grant agreement extensions may be requested based on the availability of funds, the need to extend services, recipients use of funds previously awarded, and program sustainability.
